ACL Injury Recovery: What the Rehab Process Actually Looks Like

An ACL tear is a significant injury. If you are facing ACL reconstruction, you are probably looking at nine to twelve months of recovery before returning to sport. That timeline sounds long, but it exists for good reason. Rushing back too early is the most common cause of re-injury.
The rehab process is divided into phases. Each phase has specific goals and criteria that need to be met before progressing to the next. It is not based on a set number of weeks but on how your body is actually responding.
Phase 1. The first two to four weeks after surgery focus on reducing swelling, restoring a straight leg, and beginning to get your quad firing again. You will spend time on ice, doing gentle range of motion exercises, and learning how to protect the graft while it heals. Walking without a limp is an early goal.
Phase 2. From roughly weeks four to twelve, the focus shifts to restoring full range of motion and rebuilding strength, particularly in the quad and hamstring. You will be doing single-leg exercises, building your balance, and starting low-impact cardio like cycling or swimming.
Phase 3. Weeks twelve to twenty-four involve progressive loading of the knee. Exercises become more challenging and more specific to the movements your sport requires. You start to jog, change direction at lower intensity, and work on landing mechanics.
Phase 4. From six months onwards, the focus is sport-specific conditioning and preparing for return to play. This involves running drills, cutting movements, and progressively increasing the intensity and unpredictability of training. Return to full contact sport is typically at nine to twelve months, and only when specific strength and movement criteria are met.
