Most acute back pain resolves within a few weeks without treatment. Staying active, managing inflammation, and avoiding long stretches of bed rest usually does the job.
But some back pain doesn't follow that pattern. Here's how to tell when it's worth getting assessed instead of waiting it out.
The pain has lasted more than three weeks. Acute back pain that hasn't started improving by then isn't going to resolve on its own. Something else is going on, and it needs a proper look.
The pain is getting worse, not better. Normal healing settles gradually. If your pain keeps climbing instead, that's a signal to get it checked.
The pain is moving into your leg or foot. When back pain travels down into the buttock, thigh, calf, or foot, a nerve is usually involved. People call this sciatica. It responds well to physiotherapy, but it needs the right treatment, not generic exercises pulled off the internet.
You're waking up with pain at night. Back pain that disturbs your sleep, or that feels worse in the morning than at the end of the day, can point to an inflammatory condition rather than a mechanical injury. Worth investigating.
You have numbness, tingling, or weakness in your leg. Any of these alongside back pain suggests nerve involvement and should be assessed promptly.
Your pain came on suddenly after a fall, accident, or heavy lifting. Trauma to the spine needs to be evaluated to rule out a fracture or serious soft tissue injury.
Physiotherapy for back pain works by identifying the specific structure causing the problem, treating it directly with manual therapy and targeted exercise, and teaching you how to manage load on your spine afterward. Most people see real improvement within four to six sessions.
